ABOUT US.

Paintsville Behavioral Health ("PBH") is a Therapeutic Rehabilitation Program (“TRP”) operating under the direction of a licensed clinical social worker/doctor of social work. PBH is committed to offering treatment in a patient-centered philosophy and environment of care.  Behavioral Health staff can include peer support, clinical social workers, and other behavioral health personnel.  TRP is designed to assist patients with severe mental illness in learning, mastering, and using skills that will improve their overall quality of life through individual assessment, treatment planning, and goal attainment.

A Letter From Our CEO, 
Jordan H. Pelfrey

My name is Jordan Pelfrey and I am the CEO of Paintsville Behavioral Health (“PBH”).  My dad is the owner of Pelfrey Management Group who oversees the care for residents at Venture Home.  My family has been in this business since 2017.  During this time, we have identified a gap in services for persons with severe mental illness – the need for day treatment/ TRP.  Over the last few years, we have partnered with and tried other programs but this just did not work for out patients.  We needed something better, more committed to our patients and Johnson Co.  My dad and I decided originally to open our own outpatient behavioral health program and offer TRP services.  After getting started, we felt it was better for dad to focus on PMG and Venture and me to focus on PBH.  PBH and PMG are two separate companies working in collaboration to best serve persons with severe mental illness. In order to ensure that there is no conflict of interest, that my patients get the absolute best services possible, we set up a checks and balances system to meet federal guidelines.  We make sure that every resident at Venture Home (and/or their family/guardian) understand that services at PBH are voluntary and not a condition of residency.  These patient also have a choice in providers. Several still continue to seek services with TRP programs they have been at for years and that is wonderful.  We encourage their autonomy and right to choose.  Another level of balance is that PBH is open to anyone in Johnson or surrounding rural counties who meet the criteria for admission and would benefit from services.  We look forward to growing with both PBH and the community we serve.

Dr. Cynthia R. Crase, DSW, MSW, LCSW

Dr. Crase is a licensed clinical social worker who holds a doctorate and masters degree in social work from the University of Kentucky.  She has worked in the field of social work since 2005 in many facets including mental health, addiction, foster care, intimate partner violence and medical social work.  Dr. Crase owned and operated a successful private outpatient mental health practice for six and a half years until coming to work for Paintsville Behavioral Health.  “I love working with people and have enjoyed learning about the severe mental illness population, specifically inside the rural health system.”  Dr. Crase studied and completed her capstone in the DSW on addressing barriers to quality care inside the rural mental health system.

Krystal May, MSW

Krystal holds a masters degree in social work from the University of Kentucky.  She has worked in the field of social work since 2019 in mental health and addiction treatment. “It is so exciting watching our clients grow as individuals and meet their goals. The privilege of being a part of their lives is a reward within itself.”

We invited the surveyors to evaluate how well we meet international standards for quality. The survey will tell us what we are doing well and
ways we might improve. As a result of this survey, we may earn or continue accreditation.

Strategic Plan

The mission of Paintsville Behavioral Health, Inc is to provide equitable, accessible and quality
behavioral health services to anyone in Johnson or surrounding rural counties in need of services.

Financial Disclosure Policy

When requested, Paintsville Behavioral Health shall disclose financial information to
any properly identified public funder. All other parties may be provided with limited financial
disclosure with appropriate requests.
